We now offer Flex-Pay with on-line payments only – Flex-Pay allows you to pay 25% of your total fee upon on-line registration, then three (3) additional 25% payments (automatically withdrawn in 30-day increments) for a total of 100% payment!! There is a $1.00 service charge for each additional transaction.

**NOTE - FlexPay is offered for fees that are $200 or more.  Deduction schedule subject to change as the season approaches.

SUPPORTING DOCUMENTS NEEDED FOR REGISTRANTS

Note: Participants must be division grade/age as of September 1st

Please remember that the following forms MUST be submitted, as our insurance requires it  (Want to download your forms):

  • A copy of your child(rens) Birth Certificate (for 1st-time registrants)
  • Physical Exam (signed by Physician) 
  • Medical Release (signed by Parent/Guardian)
  • Insurance Waiver

The documents MUST be presented at the 1st  day of practice to the registration committee OR at any in-park registration day (see Calendar).   There will be no entry on the fields without the submission of these forms.

ITEMS NEEDED TO PARTICIPATE (subject to slight modification)

Park will provide the following items:

Football: shoulder pads, helmet & jersey (home/away).

Cheerleading: uniforms (t-shirt and shorts).

Flag Football:
jersey only.

Parents are to provide the following:

Football: Game pants, (black), practice pants, pads (if not included with pants), cleats, black socks, mouthpiece, chin strap for helmet, athletic cup and practice jersey.

Cheerleading:
White sneakers, which are required for cheering, black leggings – for colder weather and white socks.

Flag Football:
Shorts, cleats, mouthpiece and socks.

Physical exam requirement:  All players must submit the results of a physical exam taken in the current calendar year in which they will be playing which clears them to play tackle football with their certification papers.

School grade requirement:  Along with a birth certificate requirement, all players must submit prove of their grade and school in order to be eligible to play.
